Annual Reflection Decision Record

Time and evidence

Record the period, review date, available evidence, and the difference between facts known during the period and facts learned later.

Each important source should have a date, owner, freshness, access boundary, and link to the authoritative record.

Comparison

For each material area, preserve the expectation, observation, gap, surprise, plausible explanations, revised belief, confidence, and open question.

Do not force uncertainty into a lesson. Mark claims as known, inferred, disputed, or unknown.

Decision

The record names continue, stop, reduce, increase, delegate, redesign, investigate, or defer. It also names the owner, first action, expected signal, cost boundary, and next review.

Stopped work includes the reason, remaining commitments, reusable assets, retained private records, and any condition for reentry.

Amendment

A later correction adds a dated amendment without erasing the prior decision or evidence available at the time.

Publication boundary

Private and public records remain separate. Current location, relationships, health, money, employment, career, and venture facts require independent confirmation and disclosure approval.
