Five Minute Conduct Record

The five-minute conduct record is a Venture Step method for situations where a short pause is safe. It is not for immediate danger, urgent medical needs, active abuse, a fast-moving security incident, or a deadline that requires prompt preservation, reporting, or response.

The record begins with a stop condition. If delay could increase harm or cause a required action to be missed, the person follows the relevant emergency, safety, policy, clinical, security, or legal route instead.

When a pause is appropriate, the record captures the observable event, the interpretation being added, the people affected, the duties and constraints, the next controllable action, the external action or support required, the message that must be sent, and a review time.

The design draws a boundary between facts and inference without pretending facts are always obvious. It asks the writer to name uncertainty and preserve the original source when documents, messages, measurements, or logs matter.

OSHA's complaint guidance is one example of a process where prompt reporting and accurate facts matter. The EEOC charge page is another example where formal requirements and time limits cannot be replaced by reflection. NIMH supplies the health boundary.

The method should be tested as an operating aid, not presented as an evidence-backed therapy. A useful review asks whether it improved accuracy, reduced avoidable escalation, preserved required action, clarified ownership, and produced follow-through.
