Grounding requires an authorized source manifest, preserved provenance, deliberate parsing and chunking, access-aware retrieval, answer instructions, citations, refusal behavior, evaluation, monitoring, refresh, correction, and deletion.

Retrieval success and answer faithfulness are separate measurements. A system can retrieve the right passage and still misuse it. It can also answer plausibly when no approved passage supports the claim.

Every indexed object should be traceable to an owner, version, effective date, rights basis, audience, sensitivity, source location, ingestion time, and deletion route. Changes to sources, indexes, embeddings, retrieval logic, prompts, models, permissions, or answer policies trigger reevaluation.
