The public guide takes a brief attributed lesson from The Hard Thing About Hard Things and turns it into an operating sequence. It does not reproduce the book or glorify abusive leadership.

The sequence is fact record, uncertainty, harm containment, correct channel, decision owner, communication, response, recovery, monitoring, and lessons learned.

NIST incident response guidance is a cybersecurity source. Its preparation, detection, containment, response, recovery, communication, evidence, and improvement practices are used as transferable operations disciplines, not as a universal management standard.

High-stakes channels differ. Fraud, harassment, discrimination, safety, privacy, legal, financial reporting, and regulated conduct may require protected human resources, legal, compliance, audit, security, board, union, or government routes.

An amateur investigation can destroy evidence, alert a subject, breach privacy, or expose a reporter. The guide tells readers to preserve facts and use authorized channels.

Amy Edmondson's 1999 study of 51 work teams links psychological safety with learning behavior. Psychological safety is not comfort, agreement, or freedom from consequences. It concerns the interpersonal risk of asking, reporting, admitting, and learning.

The article separates urgency from recklessness. The hard first action may be a temporary stop, evidence preservation, specialist advice, or admitting that the team does not yet know.
