The cleanest decision review begins before the outcome. Preserve the decision-time record, evaluate it on its own terms, and only then use the result as new evidence.
If the record does not exist, reconstruct it from dated documents, messages, forecasts, approvals, and testimony. Treat gaps as uncertainty rather than filling them with hindsight.

1. Define the review
Write the decision, date, owner, deadline, scope, and review question. State who commissioned the review and which consequences it can affect.
Separate learning from discipline, legal investigation, safety reporting, and regulatory processes. One meeting should not quietly pretend to satisfy every obligation.
2. Freeze the evidence
Collect only material that existed before commitment for the first review. Include the decision journal, alternatives, research, forecasts, risk record, approvals, dissent, and applicable controls.
Do not edit the original. Create an appended review record.
The outcome-bias research shows why this separation matters. Knowledge of a favorable or unfavorable result can alter judgments about the same decision process.
3. Reconstruct the decision context
Ask what the decision maker knew, could reasonably have known, and was required to know. Record missing evidence without assuming it was obvious.
Confirm the real alternatives. A team may later invent an option that was unavailable because of time, money, authority, technology, or regulation.
Record affected people, foreseeable harms, reversibility, recovery, and the cost of delay.
4. Judge process quality without the outcome
Where practical, give reviewers the decision-time packet without the result.
Ask whether the problem was framed correctly, relevant evidence was gathered, important assumptions were visible, probabilities were defined, alternatives were compared, dissent was heard, authority was valid, required controls were followed, and stop rules were credible.
Use the same rubric across similar decisions. A consistent method makes it harder to reward a lucky favorite or punish an unlucky outsider.
5. Reveal and describe the outcome
Now state what happened without interpretation. Include timing, observed benefits, harms, costs, affected people, recovery, and data quality.
Separate outcome from execution. A strategy can be reasonable while execution fails. Execution can be strong while the underlying choice is weak.

6. Compare outcome with the forecast
Was the result inside the predicted range? If the record said a failure had a 20 percent chance, one failure does not prove the estimate wrong.

Do not reduce every complex decision to one score. A forecast can be calibrated while missing severity, fairness, or an unacceptable failure.
7. Diagnose the difference
Classify what the outcome teaches. The result may reflect ordinary variance, a false assumption, missing evidence, a poor comparator, weak execution, an unanticipated interaction, a changed environment, or a control failure.
Do not label a gap luck until the team shows why the process could not reasonably have addressed it.
Do not label every surprise negligence. The standard is what a qualified person could reasonably know and do under the actual conditions.
8. Review accountability
Ask whether the decision maker acted within authority, disclosed conflicts, followed required controls, considered foreseeable harms, sought required expertise, and preserved evidence.
A sound expected-value argument does not excuse a prohibited act or unapproved risk.
Keep accountability findings tied to specific duties and evidence rather than disappointment.
9. Update the system
Every review should change something concrete or state why no change is justified.
Update an evidence requirement, probability, threshold, approval, control, owner, monitoring signal, stop rule, recovery step, training need, or documentation template.
Assign a date and owner. A lesson without an operational change is easy to admire and easy to forget.
10. Preserve the audit trail
Keep the original decision record, blind process review, outcome record, diagnosis, accountability finding, and updated rule linked but distinct.
That structure lets a later reviewer see what changed and when.
The process cannot eliminate bias. It can make hindsight harder to hide and learning easier to verify.
Review the reviewers as well. If judgments change sharply after the outcome is revealed, record that difference. It may show that the blind packet omitted material context, or it may expose the very outcome effect the sequence was designed to surface.
