The minimum decision record should identify the asset, replacement difficulty, confidentiality, environmental sensitivity, authorized users, likely attacker, expected access, response time, and required storage rules.
Physical evidence should include the exact rating and model. Anchoring should identify the substrate, fasteners, loaded weight, placement, and installer responsibility.
