Source Grounding Evidence Chain

A citation improves inspection only when the reviewer can identify the claim, open the cited passage, inspect its context, establish the source's authority and version, and decide whether the generated interpretation stays within that evidence.

The evidence chain has four independent failure layers.

LayerQuestionCommon failure
Source setIs the right authority present and current?Missing, stale, duplicated, or unauthorized material
RetrievalDid the system find the relevant passage?Wrong section, missed exception, weak ranking, or source collision
InterpretationDoes the answer faithfully represent the passage?Omitted condition, merged claims, unsupported inference, or false precision
Source truthIs the underlying material reliable for this decision?Error, outdated policy, incomplete evidence, or scope mismatch

Google's original NotebookLM announcement said grounding appeared to reduce hallucination risk and still told users to fact-check against the source. Current Gemini Notebook help exposes quoted source material and warns that experimental agentic functions can make mistakes or take unexpected actions.

The durable rule is therefore narrow. Grounding shortens the route from a question to inspectable evidence. It does not transfer authority from a selected document to every generated sentence.
